## Visión general

### ¿Quién puede usar este beneficio?
Fidelity is offering Wellthy’s Backup Care as a pilot program through December 31, 2026. This program is available to regular, U.S.-based associates regularly scheduled to work 20 or more hours per week whose office is located in Kentucky, New Hampshire, New Jersey, New York, or North Carolina.

### ¿Qué incluye este beneficio?
As an eligible Fidelity associate, you can use up to 20 backup care days during the pilot period.

#### **Definición de un día de cuidado de respaldo:**
A backup care day includes up to 10 hours of care booked in a single day. Care exceeding 10 hours in one day will be counted as two or more backup care days.

#### **Costos y cobertura:**
Fidelity cubre el costo de cuidado de respaldo hasta:  
● Up to $200 per day for providers within Wellthy’s Care Network  
● Up to $100 per day for providers within your Personal Network  
You are responsible for any costs that exceed the subsidized amount provided by Fidelity.

#### **Reembolso:**
To request a reimbursement after you’ve paid a provider, you will be required to submit proof of payment which includes: who provided the care, the date when care was provided, and the amount paid. Examples of acceptable proof include:  
● A receipt provided by a care program  
● A screenshot of digital payment (e.g., PayPal, Zelle, or Venmo)  
● A photo of a check written out to the care provider

## **Cómo empezar**

### **Activa tu cuenta Wellthy**
1. You can sign up with your preferred email address and create a password of your choice.
2. If prompted, verify that you are an eligible associate by providing your Fidelity corporate ID (a#).

Have your Fidelity corporate ID (a#) on hand, and if you think you’ll use this benefit at some point, register in advance to save time when you actually need care. Get ahead of emergencies and proactively:  
● Search Wellthy’s Care Network for providers in your area  
● Ask a Backup Care Specialist to search for you  
● Add your Personal Network in the system to have on speed dial

### **¿Tendré opciones o tendré que usar un proveedor determinado?**
You will never be required to use a particular provider for backup care. Wellthy has created a Care Network of different types of vetted options for you to choose from.

Wellthy’s Care Network of backup care options are vetted providers that go through a rigorous screening process.

Please note: Wellthy does not directly employ or oversee any provider in the Care Network or within an individual’s Personal Network.

Once the appropriate documentation is submitted, a Backup Care Specialist will review within 1 business day.

### **¿Cuánto tiempo tengo para solicitar el reembolso?**  
You have 30 days from the date of care to submit your reimbursement request.

## **Política de cancelación**

### **Si necesita cancelar la atención más de 24 horas antes de la hora de inicio de la atención programada**
As long as the cancellation occurs within the provider’s cancellation policy, you can ask your Backup Care Specialist to cancel or contact the provider directly to cancel.

- You will not be charged a day of Backup Care use.
- In some cases, providers may request payment in advance of care to hold the spot.
